Please take the time to fill out our New Client Info sheet below.  You will be asked only for the last four digits of your Social Security Number initially.  However, you will need to provide your full Social Security Number when you meet with the preparer. 

Please note that you will also need to bring a copy of all of your Depedents' Social Security cards with you.
